Add instrument.json manifest for midiboot
Declares how the model may be played from a MIDI sequencer: the entry
and defaults functions, fixed single-event overrides, the velocity (and
where applicable pitch and duration) parameter mapping, the preview
tier, and the curated knobs. Format simulated-instrument-v1; the schema
is defined and validated in proof-of-concept/midiboot.
